3 STEPS TO SUBMIT YOUR RECORDS
(Scroll to bottom if you prefer to mail your records)

Complete and Save the Patient Data Sheet 

The office has emailed you a Patient Data Sheet. There are two ways to complete it:

1) ON YOUR COMPUTER. Download Adobe Acrobat and complete your form. Save the completed form on your computer. The attached video shows this process in detail.

 

2) HANDWRITE: Print out the Data Sheet, complete it by hand, and then scan and save it on your computer. If you don't have access to a scanner, there are many free smartphone apps that will allow you to do this from your phone.

Gather Films and Records

  •  MRI Studies: The actual image files must be submitted, not just a radiology report.  If submitting a disc of images, please send only the DICOM files, but include ALL the DICOM files.  Watch the tutorial video for explanation.
  •  X-ray Images:  Four views (AP/Lateral/Flexion/Extension)
  • Current: Any films submitted should be recent, within the last 12 months
  • All body parts: if you want a consultation regarding more than one part of the spine (e.g. lumbar AND cervical) include films and data sheets for all parts.

Upload Your Files

IMPORTANT:  Please condense all your files and images into ONE zipped folder per the video instructions.
Utilize our secure file server to upload and submit your records.  All records (MRI, X-rays and completed data sheets) must be submitted at the same time.  Incomplete submissions cannot be considered.

 

 

Our record vault system will alert your Case Manager when you have submitted your records.  You also may follow up with your Case Manager if your prefer.  If you have problems completing this process, contact your Case Manager directly.
