To proceed with your free MRI review, we require specific information and imaging files from you. Below, we’ve outlined the steps and requirements in a clear, organized manner to ensure a smooth experience. Please follow the instructions carefully and complete the tasks listed in the to-do list.
Step 1: Complete the Patient Questionnaire
To provide an accurate assessment, we need a detailed understanding of your medical history and current condition.
- Action Required: Download and complete the comprehensive patient questionnaire available here: Patient Questionnaire - Cervical or Patient Questionnaire - Lumbar.
If you are seeking a consultation for multiple spinal regions (e.g., lumbar AND cervical), please submit a separate questionnaire for each area.
- Submission: Email the completed questionnaire(s) to: office@orthocareglobal.com
Step 2: Gather Your Imaging Files
We require recent MRI and X-ray images to conduct your review. These must be submitted in the correct format and meet the following criteria:
MRI Requirements:
Submit online access or the actual image files (DICOM format), not just a radiology report.
Include ALL DICOM files from the MRI CD.
Images must be from within the last 6 months.
X-ray Requirements:
Provide four views: AP (anterior-posterior), Lateral, Flexion, and Extension.
Images must be from within the last 6 months.
Multiple Regions: If your consultation involves more than one spinal area, include imaging files for each region (e.g., lumbar AND cervical).
How to Obtain Files: Contact your imaging center to request your MRI and X-ray files. They may provide Online access, QR codes, or CDs with the images.
Step 3: Prepare and Submit Your Files
Send online access login info or QR codes to office@orthocareglobal.com or follow these instructions to organize and upload your imaging files:
From CD:
Locate the “DICOM” folder on your MRI or X-ray CD.
Save it as a new folder and rename it clearly, e.g., “MRI from [Your Name] [Date of MRI]” or “X-rays from [Your Name] [Date of X-rays].”
Repeat this process for each set of images (MRI, X-rays, etc.).
Compress Files:
Zip each renamed folder into a single .zip file.
Image Submission:
Use this secure upload link to submit image files.
Here’s a checklist to ensure you complete all required steps:
1. Download and fill out the patient questionnaire Patient Questionnaire - Cervical or Patient Questionnaire - Lumbar
2. Email the completed questionnaire to office@orthocareglobal.com.
3. Contact your imaging center to obtain online access or CD of your MRI (DICOM files) and X-ray (four views) images from the past 6 months.
4. For each CD, save the “DICOM” folder, rename it (e.g., “MRI from [Your Name] [Date]”), and zip it.
5. Send online access login info or QR codes to office@orthocareglobal.com or use this secure upload link to submit image files.
If applicable, repeat steps 1–5 for additional spinal regions (e.g., lumbar AND cervical).
